Most independent ENT practices do not know their denial rate to the decimal. They know whether it feels high, and they know which payer is the problem this quarter. That is usually enough to know something is wrong and never enough to know how wrong.

We hold the practices we work with to 2.5%. Above 8%, a practice is not managing denials at all; it is absorbing them, and the absorbed cost shows up as a slow decline in revenue per visit that is easy to attribute to anything else.

The number matters less than the trend. A practice at 6% and falling is in better shape than a practice at 4% and climbing, because the second one has a cause nobody has found yet.
